Our History
Initially called Chestnut Hill Health Care Foundation, we were established in 2005 with the
proceeds of the sale of Chestnut Hill Hospital to carry out charitable activities intended to
improve the health of those living in the hospital’s primary service area.
Transitioning names but not our mission, the Foundation became Green Tree Community Health Foundation in 2008. For 13 years the Foundation worked diligently to improve the health of those affected by hunger, illness, and lack of health insurance.
In 2021, at the conclusion of a full-scale strategic planning process, the Foundation decided to change our name in order to better reflect the mission: Foundation for Health Equity (F4HE). Having listened to our community partners during the strategic planning process, we added advocacy and community engagement to our work. This allows us to better support and invest in the nonprofit partners who directly serve community members.
